About the Service

Notice regarding the service during Covid-19:

Due to Covid-19 all outpatient clinics and groups are suspended and patients referred to or waiting for this service will be held on a waiting list pending guidance that it is safe to resume these services. Virtual support will be offered by telephone or video consultations where possible and appropriate, and any patients assessed to have urgent rehabilitation needs will be escalated for management under the enhanced ESD service.

 

 

The Neuro Community Rehab team offers specialist rehabilitation to patients with a confirmed neurological diagnosis over the age of 16 in East & North Hertfordshire. Treatment is offered at home or in outpatient clinics or groups depending on need.

The team consists of Specialist Nurses for Parkinson’s Disease (PD) and Multiple Sclerosis (MS), a Neurological Navigator for Rare and Rapid Neurological Conditions, Clinical Psychology, Occupational Therapy, Physiotherapy, Rehabilitation Assistants and admin support. The team also incorporates a specialist Acquired Brain Injury Service for this group of patients. Speech and Language and Dietetic input is provided via referral to the relevant service from the team when required.

This team accepts referrals from local hospitals, GPs and wider health professionals, tertiary centres and self referrals from patients and their families. Referrals should be sent to danesbury.confidential@nhs.net.
