Neuro community therapy and specialist nursing

  • The neuro community rehabilitation team offers specialist rehabilitation to patients with a confirmed neurological diagnosis over the age of 18 in East and North Hertfordshire. Treatment is offered at home or in outpatient clinics or groups depending on need.

    The team consists of specialist nurses for Parkinson’s Disease (PD) and Multiple Sclerosis (MS), a neurological navigator for rare and rapid neurological conditions, clinical psychology, occupational therapy, physiotherapy and rehabilitation assistants  The team also incorporates a specialist acquired brain injury service and a Functional Neurological Disorder service for these groups of patients. Speech and Language therapy, Dietetics and non specialist community nursing are not available through this service but are available via referral to the relevant HCT service. 

    This team accepts referrals from local hospitals, GPs and wider health professionals, tertiary centres and self referrals from patients and their families who have been seen by the service in the past year.. Referrals should be sent to hct.neuroservice@nhs.net

Who is eligible for the service

  • People aged 18 and over with a consultant diagnosed neurological condition
  • Registered with a GP in East or North Hertfordshire

For people with:

  • Acquired brain injury (head injury)
  • Acquired spinal injury
  • Adults with neurological conditions from birth
  • Guillain-Barre
  • Motor neurone disease
  • Multiple sclerosis (MS)
  • Parkinson's
  • Stroke
  • Other rare neurological conditions

What will happen at your appointment

During your initial assessment we will complete an in-depth assessment of your current situation. By the end of your initial assessment, together, we will have identified with you the areas you would like to work on, and form a personal health care plan for your future treatment. You may be offered individual therapy sessions and/or groups. We can also provide appropriate information about other services you may find beneficial to you now, or in the future.

I feel that it was a huge privilege to be the first person in Hertfordshire to take the "Big Programme" for Parkinson's disease patients.

It was not a simple task on which to commit. When you consider I had to rely on my husband's willingness to bear the load of getting me organised and ready to start on time everyday. However, the main driving forces for both of us were George and Susie. They were steadfastly determined and encouraging, dished out with a sense of fun. We just couldn't let them down.

The results were amazing. I went from being unable to walk to walking around the outside of the Jackets Field building twice. The skills I learned are proving to be a very useful "toolkit".

Has it lasted? From the end of the course through Christmas I was much better physically and cognitively than before the course. I did get very stressed and tired after Christmas, but I did my exercises because they made me feel better. I actually walked to my daughter's house on Christmas day and back late evening (Over 400 yards one way). Coming up to New Year I went downhill and was in a bad way from New Year's day onwards, I sat for two days and had real difficulty moving. However my husband encouraged me and pushed me to make a move and I am now working my way back to fitness I think in all I only missed two days of exercises completely and did a week of one session only. I am back to exercising twice daily.

We have both been surprised at how quickly the BIG toolkit has got me back to being well.

Thank you George and Susie for my "toolkit".  I hope you will be able to offer the "BIG" course to other Parkinson's patients, it really has improved our quality of life.
