Tissue viability

  • This service offers specialist clinical advice and support in complex wound management for patients in East and North Hertfordshire. For patients in West Hertfordshire, a similar service is provided by Central London Community Healthcare NHS Trust.

    Further activity includes monitoring and management of the community service wound care products, providing expert knowledge of current treatments available and the use of advanced wound treatments.

    Tissue viability is about the maintenance of skin integrity, specialising in the management of patients with acute and chronic wounds and the prevention and management of pressure damage. We have substantial experience of wound dressings and therapies.

    Our service aims to provide:

    • clinical information
    • training and education
    • support the delivery of evidence based wound care

    A referral can be made from healthcare professionals.

    What will happen at your appointment

    We will contact you to make an appointment 

    At the appointment the specialist tissue viability nurse will carry out an holistic assessment of your wounds 

    Following which a care plan will be developed in partnership with the patient and carers 

    An agreement will be made as to the need for follow up or if follow up is required 

     

How the service is provided

  • Telephone advice
  • A joint visit with the clinician who usually provides your care in your own home, care home or at the GP practice
  • 9am to 5pm, Monday to Friday.

Who is eligible for the service

Any patient registered with a GP in East or North Hertfordshire is eligible for the service.
