Discharge Home to Assess (DH2A) is an integrated community service which supports discharges from acute and community hospitals. We support patients’ home from hospital across East & North Hertfordshire.
We accept medically optimized patients onto our pathway to receive the necessary health and social care assessments within their own home rather than these having to be completed in hospital.
DH2A provides an enablement, rehabilitative pathway for up to 21 days post discharge. In our therapy assessments and treatments we offer equipment provision, falls management, mobility assessments and post-operative rehabilitation.

We deliver a 7-day service:
8am-6pm, Mon-Fri
9am-5pm, weekends and Bank HolidaysLocation of Service

Making a referral
Referrals are accepted from:
- Acute hospitals
- Community hospitals
- Intermediate care (IMC) & Discharge to assess (D2A) beds
