West Hertfordshire
The diabetes service in West Hertfordshire is part of the Herts Valleys Integrated Diabetes Service (HIDS). This service is provided jointly by Hertfordshire Community NHS Trust, West Hertfordshire Teaching Hospital NHS Trust, and Hertfordshire Partnership University NHS Foundation Trust.
We strive to deliver high-quality specialist diabetes care in the community and closer to the homes of patients within West Hertfordshire.
The service focuses on patients with Type 1 and Type 2 diabetes in primary and community settings by the optimal use of resources and innovative models of care, which include:
- Diabetes Specialist Nurses' clinics
- Diabetes Specialist Dietitian clinics
- Community Diabetes Consultant clinics
- Diabetes education for people with Type 1 Diabetes (Dose Adjustment For Normal Eating - DAFNE)
- Diabetes education for people with Type 2 diabetes – DESMOND for people diagnosed within the last 12 months, Carb awareness for those diagnosed over 12 months ago, Living well with insulin for those who have Type 2 diabetes managed with insulin

The primary focus of our service is to strengthen overall diabetes management, significantly improve health outcomes, reduce health inequalities, and, most importantly, deliver high-quality care that is easily accessible.

How the service is provided
The service is available Monday to Friday between 8.30 am and 4.30 pm.
Who is eligible for the service
- Patients with Type 2 diabetes who require additional specialist input
- Patients with Type 1 diabetes who are not receiving care from a hospital setting and who can be managed by community care
- Patients requiring education
What will happen at your appointment
Your appointment may be on the phone or in person, depending on your need.
After treatment is complete, you will be discharged back to your GP with a clear management plan.

DAFNE
DAFNE (Dose adjustment for normal eating) is a five-day educational course for managing Type 1 diabetes. It equips patients with the necessary skills to administer the correct amount of insulin based on the amount of carbohydrates consumed.
