Diabetes service

  • There are two Diabetes services within Hertfordshire.

    West Hertfordshire

    The service is now part of the Herts Valley Integrated Diabetes Service. Please visit the website or call 01727 732073. This service is jointly provided by Hertfordshire Community NHS Trust and other NHS organisations.

    East and North Hertfordshire

    We strive to deliver high quality specialist diabetes care in the community and closer to the homes of patients within Hertfordshire.

    The service focuses on patients with Type 1 and Type 2 diabetes in primary and community settings by the optimal use of resources and innovative models of care, which include:

    • Diabetes virtual clinics
    • Diabetes Specialist Nurses
    • In practice visits led by consultants
    • Specialist dietitian input
    • Diabetes education for Type 1 diabetes Dose adjustment for normal eating (DAFNE) and for type 2 diabetes (carbohydrate awareness)

    The main focus of our service aims to strengthen the overall management of diabetes, significantly improve health and reduce health equalities and most importantly to deliver high quality care which is easily accessible.

How the service is provided

The service is available Monday to Friday between 9am - 5pm.

Who is eligible for the service

  • Patients with Type 2 diabetes who require additional specialist input
  • Patients with Type 1 diabetes who are not receiving care from hospital setting and who can be managed by community care
  • Patients requiring education (DAFNE or carbohydrate awareness)
  • Housebound patients whose diabetes is poorly controlled.

What will happen at your appointment

The patient will have an appointment lasting up to one hour with either the consultant, diabetes specialist nurse or the diabetes specialist dietitian.

If the patient requires a follow up visit they will be offered a flexible follow up option, by telephone, email, video consultation or a face-to-face clinic appointment.

After treatment is complete, patients will be discharged back to their GPs with a clear management plan.

DAFNE

DAFNE (Dose adjustment for normal eating) is a five-day educational course for managing Type 1 Diabetes. It gives patients the necessary skills to administer the right amount of Insulin for the amount of carbohydrate eaten.
