Stroke service - community

  • The community stroke team offers specialist rehabilitation to patients who have suffered a stroke aged 18 years and over in East and North Hertfordshire, up to 6 months following the date of the stroke. Treatment is offered at home or in outpatient clinics, groups or virtually depending on need. 

    The pathways available are: 

    • Early Supported Discharge (ESD) home or clinic based rehabilitation for people who have suffered a mild to moderate stroke and have identified rehabilitation goals 0-6 weeks post stroke. 
    • Supported Discharge (SD) home or clinic based rehabilitation for people who have suffered a moderate to severe stroke with more complex needs requiring multidisciplinary input. Patients are discharged from acute hospitals, or inpatient rehabilitation settings.
    • Disability Management (DM) support for patients who have suffered a severe stroke and require short term specialist support following discharge to support their optimal management in their care setting. These patients would not have rehabilitation goals. 
    • Six-month reviews for all stroke survivors in East and North Hertfordshire in partnership with the Stroke Association

    The team accepts referrals from local hospitals, GPs and wider health professionals, tertiary centres and self referrals from patients and their families who have been seen by the service in the past year. Referrals should be sent to HCNT.EastandNorthHertsESD@nhs.net

How the service is provided

  • Rehabilitation is provided by occupational therapists, physiotherapists, speech and language therapists, neuropsychologists, rehabilitation assistants.
  • Rehabilitation is delivered in the patient's home or care facility. clinic based, group and virtual provision
  • Joint working with leisure, sports facilities, social services, palliative and other specialist services 

Who is eligible for the service

  • Registered with a GP in East or North Hertfordshire
  • 18-years or over
  • Early supported discharge: Able to transfer with the assistance of one and with equipment if required, and are no longer than six weeks since the stoke occurred with rehabilitation goals
  • Supported discharge: Patient has received inpatient rehabilitation and has further rehabilitation goals, or requires specialist neurological support to enable safe discharge

What will happen at your appointment

All patients receive a welcome home visit where a holistic multidisciplinary assessment is completed.

A rehabilitation programme is then set up with the patient to enable progress towards identified goals.

Making a referral

Referrals should be received by the Stroke Co-ordinator prior to discharge from acute stroke units and inpatient rehabilitation settings.
