How the service is provided

How the service is provided

Therapy teams work with children and young people in the most appropriate setting to promote functional skills. We routinely visit early years settings, mainstream schools and specialist educational settings, children may also be seen at home or in clinic according to clinical need.

Multidisciplinary therapy teams are co-located in locality areas to facilitate the co-ordination of care for children, young people and their families. We have four teams across the county:

  • North Hertfordshire and Stevenage
  • East Hertfordshire
  • St Albans and Dacorum
  • Watford and Three Rivers

In addition to these teams we have specialist occupational therapy teams who work across the county to support with home adaptations and deliver specialist moving and handling training. We have specialist speech and language therapy teams who support dysphagia (feeding and swallowing) and multidisciplinary autism assessment pathways across Hertfordshire.

Referral form

In an ongoing effort to enhance HCT's referral-in experience for patients, The CYPT service and Project team have collaboratively undertaken the task of re-designing the CYPT digital referral form and referral automation.

Starting on the 4th March 2024, HCT will launch the updated digital referral forms. These changes have been made to help improve patient experience by providing easier access to services and streamlining the overall process. Referrals can be made to these services with the below links:

CYPT will no longer accept handwritten paper referrals. Any referrals sent using this format will be returned with a request to use the digital referral route. GPs can continue to refer into children and young people (CYP) therapies services using either the digital referral form, Ardens or DXS.

If you have any queries or experience any issues when making a referral, please contact the children's referral hub:
01923 470680 Option 2 or hct.cypspecialistservices@nhs.net

All referrals sent to the service are triaged by a therapist. The information in the referral will be used to make a decision on whether an assessment appointment will be offered. Referrals for physiotherapy must be made by a health professional. Referrals for occupational therapy and speech and language therapy can be made by education and health professionals. If your child is in full-time school you should discuss any concerns you may have with your child’s teacher. Some children can be supported in their school without a referral to the service.
