If you'd like to take part in the prize draw for your chance to win a £25 Love2Shop voucher, provide your name and contact details below the picture of the drawing or at the end of the survey, if completing online.

We'll only use contact details to notify the prize draw winners. All other information gathered in the survey is anonymous.

Prize draw rules

• There is one prize of £25 in the form of a Love2Shop voucher.
• The prize draw is open to people who have entered a drawing for the dental van design or have completed the survey, with a suggestion of a name for the dental van and a description of the design.
• The prize draw is free to enter.
• Entry to the prize draw is restricted to one entry per person.
• Multiple entries will be disqualified.
• Employees of Hertfordshire Community Trust are not eligible to take part in the prize draw.
• Entries must be submitted by 31 May 2024.
• All entrants must be a patient under the Hertfordshire Special Care Dental Service or be a child with anxiety about visiting the dentist, and a resident in Hertfordshire.
• The winner will be the best design and name, chosen by the Dental Service.
• The prize is non-transferable and no cash alternative prize is available.
• Winners will be contacted by email or telephone by 10 June 2024.
• The prize should be claimed within 3 weeks of initial notification. Failure to claim a prize will result in a redraw to select another winner.
• The use of shopping vouchers will be subject to terms and conditions of the vouchers.
• The judge's decision is final and no correspondence will be entered into.
• The name of the winner will not be disclosed, unless consent is given by the winner. 
• All personal data we collect is handled in accordance with the requirements and safeguards of the Data Protection Act.
