The Early Supported Stroke Discharge Service provides specialist stroke rehabilitation to people in their homes, who have suffered a mild to moderate stroke, and have identified rehabilitation goals. In response to Covid-19, this service has been enhanced to allow patients to be seen 8am-4pm to accommodate complex stroke referrals.
The team support the discharge of patients from the Lister Hospital Stroke Unit working closely with the stroke unit therapy team. Referrals are also received by hospitals outside of Hertfordshire.
The team have allocated social workers who are based with the team.
The service also provides a 6 month review for all stroke survivors in East and North Hertfordshire in conjunction with the Stroke Association.

- Stroke Specialist Rehabilitation is provided in the patients home for a period of up to six weeks depending on need.
- Rehabilitation is provided by Occupational Therapists, Physiotherapists, Speech and Language Therapists, Neuropsychologists, Nurses and Rehabilitation Assistants.
- Stroke patients who are able to transfer with the assistance of one and with equipment if required, and are no longer than 6 weeks since the stroke occurred.
- Registered with a GP in East or North Hertfordshire
- Has identified Rehabilitation Goals
- Stroke patients who are safe to be at home
All patients receive a welcome home visit where a holistic multidisciplinary assessment is completed.
A rehabilitation programme is then set up with the patient to enable progress towards identified goals
