Our long COVID assessment and rehabilitation service is designed to provide physical and psychological support for patients who are experiencing ongoing symptoms and complications after having the virus. At present, we offer virtual sessions only.
How the service is provided
The service is delivered by a multi-disciplinary team which includes a community GP, pharmacists, physiotherapists, pulmonary rehabilitation, respiratory nurses, speech and language therapists, dietitians, occupational therapists and clinical psychologists. The team works closely with colleagues in social care, acute hospitals, mental health and the voluntary sector to deliver a holistic approach to each patient’s care.
Who is eligible for the service
Patients who are 18 or over, with an East and North Hertfordshire GP, who have had COVID-19 and require ongoing support can be referred to the service by their GP or an acute hospital.
Please note that the service does not allow self-referrals.

Making a referral
Please note that the service does not accept self-referrals.
Referrals should be made on the referral form below, which should be completed along with the patient questionnaire. The referral form is also available on the ARDENS system for primary care colleagues. The patient questionnaire can be completed by the patient with the referrer or independently. The postal address for returning patient questionnaires is included in the service summaries below.
