Many of us are living with one long term health condition. But it's becoming increasingly common for some people to have more than one long term condition. Our Trust supports many people with multiple long term conditions so that they can manage them and live independently in their own home. Below are some of the conditions and illnesses our clinicians deal with, as well as conditions our children and young people's services advises parents on.
