ICT and Social Care Referrals

What you need to do if you work in the east and north of the county

ICT health referrals to HCT in the east and north of the county must be sent by email to the appropriate secure email address for your locality. A template referral form is available on both EMIS and SystmOne for GP practices - developed in collaboration with East and North Herts CCG GPs and HCT operational leads. The form is located in the ‘East and North Herts’ CCG folder, sub folder ‘Community Referrals’. Here are the contact details of HCT's ICT and Prevention of Admission (POA) services:

Enhanced Prevention of Admission Service

HCT’s enhanced Prevention of Admission service can help prevent unnecessary acute hospital attendances and admissions for a range of conditions by providing medical care and support for patients in their own homes or usual place of residence. The service provides responsive and urgent assessment and treatment, with same day or next day assessment. Key services and conditions covered are:

  • First dose IV/oral antibiotics to treat acute infections - UTIs, cellulitis, chest infections/pneumonia
  • Subcutaneous fluids
  • Heart failure – IV diuretics
  • COPD
  • DVT pathway
  • Point of care testing for infections and DVT markers
  • ECG - when 999 response not required
  • End of life and specialist palliative care
  • Support for care homes with COVID-19 outbreaks
  • Medication reviews by pharmacists
  • Falls, urgent mobility assessments and rapid decline in functional ability

The service is open to patients with a GP in East and North Hertfordshire who are aged 18 and over, both with and without COVID-19.

How to refer to the service

  • Call the HCT East and North Referral Hub on 0300 123 7571 – choose option 4 to discuss a patient directly with a GP based in the Hub
  • The service runs from 9am to 8pm seven days a week
  • Referrals for same day assessment accepted up to 6pm
  • Referrals can be accepted by any healthcare professional who has assessed the patient

Referrals to IPA

Please be assured that any referrals sent to the IPA will be redirected to the Trust and will be picked-up by our ICTs.
