ICT and Social Care Referrals

The way we receive referrals into our Integrated Community Teams (ICT), as well as social care referrals to Hertfordshire County Council (HCC), is changing.

Referrals made to the Integrated Point of Access (IPA) email address will not be processed after 1 April 2017 and GPs and other health staff who refer into the services will need to follow the new processes outlined below.

What you will need to do if you work in the east and north of the county

ICT health referrals to HCT in the east and north of the county must be sent by email to the appropriate secure email address for your locality.

A template referral form will be uploaded in advance and ready for use on both EMIS and SystmOne for GP practices. Practices will be advised when this will happen. The new form has been developed in collaboration with East and North Herts CCG GPs and HCT operational leads. The form will be located in the ‘East and North Herts’ CCG folder, sub folder ‘Community Referrals’.

This new way of working will enable practices to pre-populate the referral forms with information which will reduce time spent on the referral process. Please see the contact details of HCT ICTs, HomeFirst, and Rapid Response teams below:

What you will need to do if you work in Herts Valleys

ICT health referrals to HCT in the west of the county must be completed using the revised Locality ICT Referral Forms (available on DXS from 1 April 2017) to the relevant secure email address (see below) for your locality.  These forms will also be available on the intranet in MS Word format to download if required. Important Fax Changes

Please note that under guidance from our Senior Information Risk Owner (SIRO) the Trust will now no longer accept any information sent via fax from 1st April 2017. If you have any concerns, please contact your local ICTs directly.

Old Referral Forms

The Trust will initially accept the use of old IPA forms to ease the transition. We do however hope to phase out the use of these forms as quickly as possible.

Referrals to IPA

Please be assured that any referrals that may still be sent to the IPA will be re-directed to the Trust and will be picked-up by our ICTs.
