The HAH service for East and North Hertfordshire provides hospital equivalent care in the patient’s own home*. Meaning acute hospital stays or attendance can be avoided.
HAH offers in person nursing care, therapy and remote monitoring. All overseen by a team of health professionals.
The HAH service provides responsive, urgent assessment and treatment. Including same or next day assessment. It is delivered by a team of GPs, nurses, pharmacists, and therapists. All have easy access to advice and care from dieticians, palliative care, the community respiratory team and secondary care advice as needed.
The HAH service is available 8am-8pm seven days a week. Patients can remain under HAH for up to two weeks.
*This includes care homes.
Hospital at Home capabilities
Many patients live with multiple conditions and would benefit from a personalised approach, combining remote monitoring with in-person care when required. HAH offers a range of capabilities that can be safely delivered in the patients home. This means patients in the community can remain at home avoiding hospital admission or attendance. While those currently in acute hospitals can be safely discharged to continue their treatment at home.
Where remote monitoring is required patients will be provided with equipment and shown how to use it by Doccla. Consent for gathering their data is required.

The service is open to patients with a GP in East and North Hertfordshire who are aged 18 and over.
Referring to HAH
The service is available through referral only. Referrals can be accepted from any healthcare professional who has assessed the patient. There are two options to refer to Hospital at Home:

Referral criteria
Referrals are accepted up to 8pm - seven days a week. After 6pm referrals may be triaged for assessment the following day.
Inclusion criteria:
Aged 18 years or over
Registered with a GP in East and North Hertfordshire
Exclusion criteria:
Self-referrals
People under 18 years of age
Pregnancy
Patients who can be clinically managed by their own GP or in a Minor Injuries Unit
Social needs only, where the patient has no healthcare needs.
Patient is not safe to be managed in the community and needs admission to an acute hospital.
Patients who are unsafe alone in their property between visits or overnight.
MND, PD, MS or other neurological patients who go through the specialist Neuro POA pathway.
