East and North Herts NHS Trust and Hertfordshire Community Trust Therapy Staff jointly
run two consultant led multidisciplinary Falls Clinics per week.
The purpose of the clinics is to provide MDT assessments of a patient’s falls risks, in line with local and national guidelines. Referrals can be made from the clinics for further medical investigations with follow up and provision of any home based therapy prevention measures, resource/equipment provision and rehabilitation.
There are 2 clinics running which are based out of:
The Lister Hospital in Stevenage on a Wednesday afternoon 13.00-16.00, led by Dr E Lines and Dr C Rippingale, and
The New QEII Hospital in Welwyn Garden City on a Friday morning, 09.00-12.00 led by Dr D Jain.
The MDT team consists of 3 Consultants who share the Clinics, a Senior Occupational Therapist, a Senior Physiotherapist and a Technical Instructor who provides ECG and BP recording.
- Patients over the age of 65, registered with an East and North Herts GP
- Patients who have a recent history of falls in the community
- Patients recently discharged from the acute or A&E following a fall.
- Patients reporting disturbances in balance and gait that are deemed to be at high risk of falls by their GP.
Each patient who is referred will attend at the start of the clinic and leave following assessment from each clinician. The clinic concludes with a MDT meeting to discuss individual patient’s assessments and collate any ongoing investigations/management plans. Following attendance at the clinic a letter will be sent to the patient and their GP detailing the outcome and planned interventions/ investigations.
