Hertfordshire Community Trust will be operating a domiciliary phlebotomy service in the Upper Lea Valley, Lower Lea Valley and Stort Valley and Villages localities. This service will provide a high quality phlebotomy service to patients in these areas who are housebound or shielding.
The service is made up of:
- 3 phlebotomists to carry out visits
- 1 nurse to triage and schedule visits
- Locality/service manager support
- Admin support via referral hub
The service is for patients:
- aged 18 or over,
- registered with a GP in the areas above
- who are unable to access the phlebotomy clinics due to being housebound or shielding
- who do not require blood tests for anticoagulation or INR (this will continued to be managed by ICT teams)
- who are not receiving care from the HCT Prevention of Admission team
Referrals into this service will be through the HCT Referral Hub via the normal route
(HCT.StortValleySPOC1@nhs.net, HCT.LowerLeaValleySPOC@nhs.net, HCT.UpperLeaValleySPOC@nhs.net, tel: 0300 123 7571), with an accompanying appropriate phlebotomy referral request form. Please note that referrals without a Phlebotomy request form will not be accepted. The team at the referral hub will then direct the referral to the phlebotomy team for the visit to be arranged.
