Enhanced Prevention of Admission Service

HCT’s enhanced Prevention of Admission service can help prevent unnecessary acute hospital attendances and admissions for a range of conditions by providing medical care and support for patients in their own homes or usual place of residence. The service provides responsive and urgent assessment and treatment, with same day or next day assessment.

We provide a seven day service allowing clinicians to spend time developing holistic and individualised management plans adding frailty expertise as needed. We have a multidisciplinary team of GPs, nurses, therapists and pharmacists with easy access to advice and care from dieticians, palliative care, the community respiratory team and secondary care advice as needed. We can keep patients under our care for up to two weeks.

  • First dose IV/oral antibiotics to treat acute infections - UTIs, cellulitis, chest infections/pneumonia
  • Subcutaneous fluids
  • Heart failure – IV diuretics
  • COPD
  • DVT pathway
  • Point of care testing for infections and DVT markers
  • ECG - when 999 response not required
  • End of life and specialist palliative care
  • Support for care homes with COVID-19 outbreaks
  • Medication reviews by pharmacists
  • Falls, urgent mobility assessments and rapid decline in functional ability

The service is open to patients with a GP in East and North Hertfordshire who are aged 18 and over, both with and without COVID-19.

When accepting a referral we consider:

  • Is it safe and appropriate to keep this patient at home?
  • Does this fit with the patient’s wishes?
  • Will our intervention prevent a hospital admission?
  • What is the likely diagnosis? (With the exception being delirium we are not a diagnostics service)
  • Has the referring clinician assessed the patient either face to face or via video?
  • Can our service add value that would not be possible within usual primary care?
  • Would this intervention be better delivered by another service?
  • What is the treatment escalation plan?

How to refer to the service

  • Call the HCT East and North Referral Hub on 0300 123 7571 – choose option 4 to discuss a patient directly with a GP based in the Hub
  • The service runs from 9am to 8pm seven days a week
  • Referrals for same day assessment accepted up to 6pm
  • Referrals can be accepted by any healthcare professional who has assessed the patient
