Trust work to increase awareness of end of life care nominated for prestigious Macmillan excellence award

Hertfordshire Community NHS Trust’s (HCT) Macmillan Clinical Education Team was recently shortlisted for a prestigious Macmillan Professionals Excellence award, which recognises inspirational work in training and educating staff to deliver end of life care.

Kemi Koleoso, Macmillan Clinical Education lead at HCT, and her team were nominated in theInnovation Excellence category, which celebrates the people whose vision and commitment has made a lasting difference to the quality of services offered to people affected by cancer, who are nearing the end of their life.

Since their launch in 2016, the team have played a crucial role in helping clinicians improve care planning for Hertfordshire patients who are nearing the end of their life.

Kemi said: “Initially, some staff don’t recognise their role in providing end of life care because they don’t think they see end of life patients in their clinics. It’s about showing them that they do, and explaining that people at the end of their lives may be active and attending clinic sessions, and therefore need signposting to further care.”

The effects of the programme are already being felt around The Trust, securing a lasting legacy for the future.

Kemi added: “The education team has manged to make end of life care ‘everybody’s business’ by delivering training to all clinical teams in the Trust. This can make a tangible difference to both their practice and their patients.”

Delivering innovative and tailored training for individual clinical teams, as well as introducing a network of champions across all clinical services, has served to highlight end of life care across the entire Trust and Hertfordshire as a whole.

Kemi went on to add: “Our End of Life Care Champions programme is a key innovation. We have managed to recruit champions from all our clinical teams. We meet with them quarterly for teaching, updates and support.The champions in turn promote end of life care within their teams and ensure the quality of care given to patients is of a high standard.” 

The Macmillan Professionals Excellence Awards were held on 16 November in Heathrow. Hertfordshire Community NHS Trust’s (HCT) Macmillan Clinical Education Team was one of five shortlisted finalists.
