Information for health, social care and education professionals

Welcome to our pages for referrers and operational partners. The information in this section details our service offers during the response to Covid-19, including relevant referral forms and contact information.

Our overall service offer during Covid-19

We are following the Prioritisation within Community Health Services guidance from NHS England and Improvement which was published early on in the response to Covid-19. This guidance details those community health services that should be stopped or reduced to "urgent-only" in order to increase capacity to respond to Covid-19. Referrals for these services can still be made in the usual way, and we are prioritising patients and service users who need urgent care. In addition to implementing the national guidance, we have developed enhanced services to respond to the local need in East and North Hertfordshire.

Our services for adults in East and North Hertfordshire  Our services for children and young people across Hertfordshire

Our services for adults in East and North Hertfordshire are focused on:

  • Prevention of admission
  • Keeping people safely care for at home/their normal place of residence
  • Supporting and enabling early discharge home from an acute hospital
  • Dedicated support for care homes, including those specifically allocated to caring for patients with Covid-19
  • Using multidisciplinary teams to provide a wider range of support and care for patients at home

Many of our services for children, young people and families have been suspended in line with national guidance for community Trusts. This has allowed us to redeploy clinicians into adult services where appropriate. We are still providing core services and support, including:

  • Public Health Nursing Service (School Nursing and Health Visiting) - families can access support online and by telephone. Families have been individually contacted to advise them on the support available
  • PALMS and Step2 (CAMHS) services continue to support mental health and wellbeing
  • Schools and Families Advice Line (SFAL) - emotional wellbeing advice and early help to children and young people, school staff and families
  • Expanded Children's Observation and Rapid Assessment Service (CORAS) in West Hertfordshire
  • Enhanced community nursing service in West Hertfordshire
